DSA 2021 APPLICATION
DSA annual dues provide members with a listing on the DSA website and representation to numerous travel and bridal shows. To submit a new membership or renew your membership please follow the below instructions:
· Complete this form and send it back to DSA by either regular mail or email.
· Mailing Address: 1350 E. Flamingo, #170, Las Vegas, NV 89119, along with a check or credit card information for $100.00
